Consent and Waiver

This Consent and Waiver is signed and delivered by the undersigned participant in events and programming (“Programming”) hosted by Cure Experience Services LLC (“Cure”).

  1. Photographs. I acknowledge that photographs will be taken at Programming, and I hereby irrevocably grant permission to Cure, without expectation of compensation or remuneration now or in the future, to photograph and otherwise capture my image, and to use my image and likeness on Cure’s website or other publications promoting Cure programming, including newsletters or features by Cure, or in any internal or external communication, currently existing or later created, in any medium, including publicly available platforms such as Cure’s website, LinkedIn and YouTube for any legal purpose. I hereby release Cure and its owners, officers, directors, managers, agents, employees, affiliates and successors (collectively, “Cure Parties”) from all liability in connection with such usage.

  2. Videotaping. I understand and agree that Programming will be videotaped, and I hereby irrevocably grant permission to Cure, without expectation of compensation or remuneration now or in the future, to photograph and otherwise capture my image, and to use my image and likeness in any internal or external communication, currently existing or later created, in any medium, including publicly available platforms such as Cure’s website, LinkedIn and YouTube, for any legal purpose. I hereby release Cure Parties from all liability in connection with such usage.

  3. Recording. I understand and agree that Programming will be recorded, and I hereby irrevocably grant permission to Cure, without expectation of compensation or remuneration now or in the future, to record and otherwise capture my voice, and to use such recording in any internal or external communication, currently existing or later created, in any medium, including publicly available platforms such as Cure’s website, LinkedIn and YouTube, for any legal purpose. I hereby release Cure Parties from all liability in connection with such usage.

  4. No Confidential Information. I agree not to disclose any confidential information or material non-public information to Cure or any other person in connection with my participating in Programming. I understand Cure and Programming participants are under no duty to keep confidential any information I disclose.

  5. Waiver. I understand and agree that my participation in Programming and any activities organized in connection therewith, including activities hosted by any third party, is voluntary and is undertaken at my sole risk and responsibility. I hereby release and agree to hold harmless Cure Parties from and against and all liability for bodily injury and/or property damage in any way resulting from or related to my participation in Programming.

  6. Eligible to Receive Gifts. I confirm that if I accept any gifts provided to Programming participants, I will be in full compliance with any restrictions limiting or prohibiting my receipt of gifts.

This Consent and Waiver is governed by the laws of the State of New York, without regard to principles of conflicts of laws.
